All authors/faculties are required to sign the following documents:
- Content Provider / Author Agreement
- Conflict of Interest Declaration
- Affidavit of Image Authenticity
- Statement of Policy Regarding Assurance of Balance, Disclosure, Non-Commercialization
- W-9 Form
